ANTICOAGULATION MANAGEMENT SERVICE

The Anticoagulation Management Service (AMS) is for patients taking Coumadin® or warfarin. We provide patient education about your medication and also monitor your therapy with a finger stick blood test.

The goals of this program include:

  • Monitoring your lab values (PT, INR) and adjusting your Coumadin® dose as needed.

  • Providing you with appropriate education and teaching, including diet and exercise education

  • Reviewing all medications and providing you with a resource for answering questions pertaining to Coumadin® and other medications.

Coumadin® is a medication used to thin the blood to prevent clots from forming in patients with an irregular heartbeat, history of stroke or clotting disorders, and other conditions.
